Kelly Park Homeowners Association

CLUBHOUSE

When you are planning to use the clubhouse for an event you must obtain and complete the Clubhouse Application Form and sign that you agree to all the following rules. Use of the clubhouse does not include exclusive use of the pool or spa.

The following guidelines must be followed:

1. Only a homeowner of KELLY PARK HOMEOWNERS ASSOCIATION may reserve the clubhouse. Tenants must have the owner of their unit make the reservations and the owner will be held responsible.

2. The homeowners/resident reserving the clubhouse must be present at all times during the event.

3. No reservation will be accepted more than 45 days in advance. (If there are two reservations for the same day, the party that has used the clubhouse the least in the previous six months will receive the reservation.)

Note: The 45 day advance reservation limit or fees do not apply to the Kelly Park HOA, which may make reservations in advance for board meetings, holiday events, etc.

4. This form must be signed and returned to AMS Realty, Inc., 2323 Portola Road, #150, Ventura, CA 93003 along with a $50.00 reservation fee (non refundable) plus a $200.00 deposit. The $200.00 deposit is refundable only if the clubhouse is left in a clean, orderly and undamaged condition with removal of all trash and debris from the clubhouse area. The deposit may be used to pay for necessary cleaning, repair of damages, or replacement of missing items. (Two separate checks made payable to KELLY PARK HOMEOWNERS ASSOCIATION).

5. The clubhouse must be vacated by 10:30 p.m. on weekdays and 11:30 p.m. on Friday and Saturday. All set-up and cleaning must be done the DAY OF THE RENTAL. You may not enter the clubhouse a day early to decorate or come back the next day to clean! THE HOURS OF YOUR RESERVATION ARE THE ONLY HOURS YOU ARE AUTHORIZED TO BE IN THE CLUBHOUSE.

6. Any events held in the clubhouse which result in boisterous and/or unruly conduct as determined by the Board of Directors, may result in monetary penalties and/or suspension of the clubhouse privileges.

7. The clubhouse may not be used for illegal, commercial, or political purposes. There are to be no signs or posters or passing of literature in, about, or facing the common area.

8. KELLY PARK HOMEOWNERS ASSOCIATION is not liable for any misuse or misconduct of the facility.

9. The person reserving the Clubhouse will sign this statement: "I have been given a copy of the Clubhouse Inventory and Helpful Hints for Clubhouse Rental. I have read the rules governing the KELLY PARK HOMEOWNERS ASSOCIATION and hereby agree to abide by them. I understand that any infraction of the rules may result in monetary penalties. I further understand if there are damages or clean-up expenses after my use of the clubhouse, my check will be deposited and the amount will be deducted from my deposit."

*PLEASE NOTE: If the clubhouse key is not received by AMS within seven (7) working days after the event, the owner will be charged the cost to re-key the clubhouse (approximately $50.00).

Kelly Park Clubhouse Helpful Hints/Inventory

The following is a checklist for you, to ensure the clubhouse is left in proper order because it is inspected after each use. If left unclean, debris not properly removed and/or damaged your $200.00 deposit will be used to pay for all related costs.

1. Please bring all necessary cleaning products with you to clean the kitchen, floor, counters, refrigerator, etc. THE CLUBHOUSE MUST BE LEFT CLEAN WHEN YOU LEAVE!!! NOT THE NEXT MORNING!!!! NO EXCEPTIONS !!!!!

2. You must vacuum the carpet after your event.

3. You must bring trash bags with you to clean up. Trash must be hauled back to your residence – NOT PLACED IN THE POOL AREA OR OUTSIDE THE DOOR - as there is NO trash pick up for the clubhouse.

4. You must remove all trash from both inside and outside of the clubhouse! If your guests use the trash bins inside or outside the clubhouse make sure to empty them out before leaving.

5. All lights must be turned off and all doors securely locked when leaving the premises.

6. The clubhouse inventory will be checked to insure all Kelly Park property is left inside the clubhouse. The inventory consists of:

1 Microwave
1 Oven/stove
1 Refrigerator
1 Sofa
1 Love seat
2 Chairs
Table Lamps
End tables
Coffee Table
1 Eight-foot oak table
6 Dining table chairs
40 Thickly padded folding chairs
6 Six-foot long folding tables
